Southerlin Family Eye Care is accepting new patients. We strive to make your experience at Southerlin Family Eye Care as convenient and pleasant as possible. The forms required at the time of your first visit are available below to provide the convenience of completing them in the comfort of your home prior to your appointment. Please print and complete them and bring them to your visit.
Insurance
We accept VSP, EyeMed, Community Eye Care, and Physicians Eye Care Plan as full-service providers (meaning we are contracted to provide exams and also glasses and/or contacts to these patients). We also accept Superior Vision as an exam and contact lens only provider (we are not contracted to provide glasses to these patients but can offer perform the exams needed and give guidance as to great places nearby that can get these patients their glasses in-network!). Please contact us regarding any questions you may have regarding your insurance coverage.
We are also medical providers for Blue Cross Blue Sheild, United HealthCare, and Medicare. We are always adding more plans, so contact us if yours isn't listed!

Eyecare Emergencies
Please note that patient care is at the foremost of our practice, and due to our hours and small staff, we refer many red eyes and ocular medical conditions to other experienced local providers in order that our patients will receive proper and timely care. If you ever notice a sudden loss in vision (one eye or both), extreme pain in or around your eye, flashes of light, or anything that you feel is out of the ordinary, please contact a local ophthalmology group immediately. These groups are listed under “Physicians-Ophthalmology” in the phone directory.
Holiday and Weather Closings
In addition to the holidays that fall on a Monday (when our office is always closed), we are closed New Year's Day, Good Friday, Independence Day, Thanksgiving and the Friday after Thanksgiving, Christmas Eve, Christmas Day, the day after Christmas, and New Year's Eve.
In case of inclement weather, please note that we will typically follow Greenville County Schools schedule of closing and/or delays. Closings and/or delays will be posted to our Facebook page, on our voicemail at our office, and on WYFF-4. If your appointment time is missed due to a weather closing or delay, please note that we will try to contact you to reschedule as soon as it is possible.
